Please note: Athletic Registration and Physical Form must be completed in order to participate.

Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination take prior to April 1 is valid only for the remainder of that school year and the following school year. Once you have the physical form completed you may either upload with your registration or bring it to Mrs. Viegut in the LMHS office. To ensure clearance for participation, physicals should be completed and turned in to the athletic director no later than 3 school days prior to the first day of practice/tryouts. 

        To ensure clearance to compete, the athletic fee must be turned into the front office to Nikki Viegut prior to the first game, meet or match. High school athletic fee is $25 per year and middle school athletic fee is $15 per year.
